Medical Information

Please review the following information regarding medical issues

PHYSICALS

All Kindergarten, seventh graders, and out-of-state transfer students need to have a medical physical within six months prior to the start date of school.  All students going out for athletics must also have a physical after May 1 of the subsequent year they plan to participate in athletics.  Forms are available at the office.

MEDICINE

Whenever possible, your child should be given medications outside school hours.  In the event that it is necessary that the child have medication at school, written permission from the parent or guardian requesting the school to administer the medication is required.  MEDICATION MUST BE PROVIDED TO THE SCHOOL BY THE PARENT/GUARDIAN IN THE PHARMACY OR MANUFACTURED LABELED BOTTLE AND GIVEN TO THE SECRETARY.  REPACKAGED MEDICATIONS WILL NOT BE ACCEPTED

DETAILED INSTRUCTIONS ON THE DRUG NAME, DOSE AND INTERVALS TO BE TAKEN ARE NECESSARY.  FORMS ARE AVAILABLE AT THE OFFICE OR ONLINE.

All medicine is to be stored and administered in the office.  Students are not to carry medication with them or store it in their desks or lockers, for their security and the safety of others.

STUDENT ILLNESS

Parents will be notified when a child becomes ill at school.  Conditions requiring a student to be sent home include: temperature at or greater than 100 degrees, vomiting, diarrhea, unexplained rashes, head lice or any condition which presents a health risk to others, or prevents meaningful participation in the educational program.  STUDENTS MUST BE FEVER-FREE FOR 24 HOURS BEFORE RETURNING TO SCHOOL.

A STUDENT MAY NOT RETURN TO SCHOOL UNTIL 24 HOURS AFTER THE LAST EPISODE OF VOMITING OR DIARRHEA.

 

ABSENCES

Students are allowed five absences per semester, (including medical, dental visits, etc.).  If a student needs to miss school for a doctor, dental, therapy or other medical reason, please bring a signed note from your doctor’s office after the visit.  Parents must send a note prior to the medical visit so that we can assign the student a pre-makeup slip.  On the sixth absence, students are subject to a credit reduction.

If a student is ill, please call the office right away in the morning.  We also need a note when the student returns, in the parent or guardian’s handwriting stating why the student was absent.
